PSA testing is routine for men over 50, and elevated or rising levels cause real anxiety. Saw palmetto is one of the first things men research. But can it actually reduce PSA — and should you try to lower it with supplements?
PSA (prostate-specific antigen) is produced by prostate cells. Elevated levels can indicate prostatitis, BPH, prostate cancer, or recent physical stimulation of the prostate. PSA alone cannot distinguish between these causes — clinical context and additional testing are essential.
Saw palmetto inhibits 5-alpha reductase (5AR) — the same enzyme targeted by prescription drugs finasteride and dutasteride, which are well-known to reduce PSA by approximately 50%. Saw palmetto inhibits 5AR less potently, but the mechanism is identical. Studies show therapeutic-dose saw palmetto can modestly reduce PSA by approximately 10–25% in men with BPH.
A 2001 study in Prostate found 160mg twice daily for 6 months reduced PSA by an average of 22% in men with BPH, correlating with improvements in urinary symptoms — suggesting genuine reduction in prostate inflammation and glandular activity.
The large STEP trial using 320mg daily found no significant PSA change over 12 months vs. placebo — likely due to different extract standardization.
Saw palmetto's PSA effect is highly dependent on extract quality. Poorly standardized products (below 85% fatty acids) show little effect. High-quality extracts standardized to 85–95% fatty acids show more consistent results.
There's an important difference between PSA reduction from genuinely healthier prostate tissue (legitimate) versus artificially suppressing PSA through hormone manipulation (potentially dangerous for cancer screening). Because saw palmetto's mechanism overlaps with pharmaceutical 5-ARIs that mask PSA, transparency with your physician is essential.
